PRP has been increasingly offered despite high cost, limited AAOS endorsement, and lack of FDA approval; moreover, few rigorous RCTs directly compare PRP with CS—an effective, cost-efficient option with benefits lasting up to ~3 months. This trial addresses the short-term comparative effectiveness gap to inform expectations and value-based decision-making.
53 patients with symptomatic knee osteoarthritis were randomized to receive either platelet-rich plasma (PRP) or corticosteroid injection with triamcinolone acetonide. The primary outcome of interest was pain, measured on a Visual Analog Scale (VAS). Secondary outcomes included knee-specific and functional outcomes.

At 6 weeks, patients who received corticosteroid reported significantly greater reductions in pain compared to the PRP group. Clinically relevant improvements were observed in the corticosteroid group but not the PRP group.

At 3 months, both groups reported similar levels of pain improvement.
Bottom line. Corticosteroid appears to provide significantly greater pain reduction at 6 weeks post-injection compared to PRP. This study suggests that expectations regarding the clinical utility of PRP should be tempered.
